Location: Holywell, Flintshire | 37 hours a week

Different days each week - Monday - sunday, Three 12 hour shifts.

This role matters because compassionate, reliable support underpins safe, dignified care for people who rely on community and housing services. As a Support Worker based in Holywell you will make a practical difference each day by helping people live as independently and well as possible.

Purpose of the role

The purpose of this role is to provide day-to-day care and practical support to residents and service users in a community or housing setting. You will work with colleagues to deliver person-centred support, follow care plans, maintain safety and promote wellbeing.

Key responsibilities

• Deliver personalised support and assistance with daily living tasks according to care plans.
• Support medication administration where required and in line with local procedures.
• Observe, record and report changes in a service user’s condition, behaviour or needs.
• Promote independence, dignity and choice for service users in all interactions.
• Work collaboratively with colleagues, visiting professionals and family members to coordinate care.
• Maintain accurate and confidential records and complete required documentation.
• Follow safeguarding procedures and contribute to a safe, clean environment.

Essential skills and experience

• Experience of working in health, nursing or social care settings, or demonstrable transferable experience.
• Practical caring skills and the ability to support people with personal care and daily tasks.
• Good communication skills and an ability to build rapport with service users and colleagues.
• Basic record-keeping and observational skills to report changes accurately.
• Ability to follow care plans, policies and safeguarding procedures.
• Right to work in the UK and willingness to work rota patterns typical for care roles.

Desirable skills and experience

• Previous experience in supported housing or community care services.
• Training in medication administration, moving and handling, or positive behaviour support.
• Full driving licence and access to a vehicle where the role requires travel between sites.
• Experience working with people with long-term health conditions, mental health needs, or learning disabilities.

Qualifications

• Relevant vocational qualifications such as NVQ/SVQ/City & Guilds in Health and Social Care are desirable where the role requires them.
• Mandatory checks typical for this sector (for example, criminal record checks) will be required where applicable.
